Job summary

Pinnacle Family Services is seeking a Family Centered Treatment (FCT) Specialist to work with children, adolescents, and families in home and community settings. You will guide families through the four phases of the FCT model, develop personalized service plans, and coordinate with medical, behavioral health, school, and community partners to ensure holistic care.

Requirements include a Bachelor’s Degree in Human Services (or equivalent experience) and a valid driver’s license; FCT

Qualifications

• Bachelor's Degree required — in Human Services with a minimum of two years of experience, or in a non‑Human Services field with a minimum of four years of experience
• FCT Certification is a plus, though not required — we'll help you get there
• Must meet qualifications as a Qualified Professional (QP) with experience serving children and adolescents with a primary mental health diagnosis

Responsibilities

• Deliver Family Centered Treatment services primarily in home and community settings
• Guide children and families through the four phases of the FCT model, setting goals and building skills
• Serve as central point of coordination for each family's Treatment Team with partners
• Develop, implement, and refine individualized Service Plans with families
• Connect families to community resources, referrals, and aftercare supports
• Maintain timely documentation in the Electronic Health Record reflecting quality of care
• Participate in Quality Assurance activities to strengthen programs

Education

Bachelor's Degree in Human Services or equivalent experience
