Job summary

CBH Care, a nonprofit provider of community mental health services, is seeking a Per Diem Community Support Services Counselor. The role offers flexibility and the chance to contribute to a mission-driven organization.

Responsibilities include IMR and crisis intervention, assisting with daily living skills, implementing treatment plans, and traveling to consumers' residences as required. A valid driver’s license is required.

Qualifications

• BSW in social work or BA/BS in psychology.
• One year of direct care experience preferred.
• Valid Driver's License.

Responsibilities

• Provides IMR and crisis intervention services.
• Assists with food shopping, laundry and daily living skills.
• Implements prescribed treatment plan and assists in the development of IRPs.
• Must be able to work independently and travel to consumers’ residencies as required.
• Monitors medication in the consumers home.
