Job summary

Benefis Health System in Montana seeks a social service professional to provide psychosocial assessments, navigate housing and financial resources, and coordinate care planning for elderly or disabled beneficiaries living at home or in the community.

You will work within the Care Management team, addressing mental health needs, long-term care planning, and collaboration with a multidisciplinary group to deliver person-centered, community-based care.

Qualifications

• Bachelor’s degree in social work or related field.
• Three years of social work experience in a health care setting preferred.
• Knowledge of case management methods, practice, and procedure.
• Knowledge of diagnostic and crisis intervention skills.
• Knowledge of issues and needs of long-term care consumers.
• Prior knowledge of managing a budget.
• Knowledge of human behavior, disabilities, and the aging process.

Responsibilities

• Provide psychosocial assessments, mental health support, housing navigation, financial stability guidance, and connect clients with community resources.
• Coordinate psychosocial services as part of the Care Management team to improve quality of life and emotional well-being.
• Collaborate with multi-disciplinary Care Management Team to plan, assess, and monitor services for beneficiaries living at home or in the community.
