NAFI Connecticut, Inc. is seeking a Home Based Case Manager to provide in-home case management services to at-risk youth and families across Rhode Island, with a statewide scope and based in Warwick.

Our Parenting with Love and Limits Program (PLL) is looking for Home Based Case Manager to provide case management services to adolescents that are either removed from their home or are at risk of removal due to high-risk behaviors. PLL is an evidence-based model which provides case managers with high levels of training including bi-weekly group supervision and consultation. Case Managers work with a clinician on a caseload of 15 clients and families, do weekly home visits with each family, support clinicians in facilitating the parenting groups and participate in training and deliver services with fidelity to the PLL model. Current position is statewide, based in Warwick and will require a flexible schedule with non-traditional hours.
