Job summary

NAFI Connecticut, Inc. in Warwick, RI seeks a Home Based Case Manager to provide in-home case management for adolescents at risk or removed from home.

You will work with a clinician on a caseload of 15, conduct weekly home visits, and support parenting groups within the PLL program. Responsibilities include connecting families to local services, collaborating with a multidisciplinary team, and delivering services with fidelity to the PLL model.
