NAFI Connecticut, Inc. in Warwick, RI seeks a Home Based Case Manager to provide in-home case management for adolescents at risk or removed from home.

You will work with a clinician on a caseload of 15, conduct weekly home visits, and support parenting groups within the PLL program. Responsibilities include connecting families to local services, collaborating with a multidisciplinary team, and delivering services with fidelity to the PLL model.

Job description

NAFI Connecticut provides programs, consultation, and support services in a variety of settings across Connecticut, New York and Rhode Island. We work with adults, children, and families to increase communication and social skills, improve relationships, increase circles of support, and provide therapeutic services based on best-practice research. Our Parenting with Love and Limits Program (PLL) is looking for Home Based Case Manager to provide case management services to adolescents that are either removed from their home or are at risk of removal due to high-risk behaviors. PLL is an evidence-based model which provides case managers with high levels of training including bi-weekly group supervision and consultation. Case Managers work with a clinician on a caseload of 15 clients and families, do weekly home visits with each family, support clinicians in facilitating the parenting groups and participate in training and deliver services with fidelity to the PLL model. Current position is statewide, based in Warwick and will require a flexible schedule with non-traditional hours.

Responsibilities Include:

Salary: $55,000 per year
