Job summary

SBHonline is seeking a compassionate Case Worker to provide comprehensive support to Holocaust Survivors enrolled in our Homecare Program. You will assess client needs, coordinate services, and advocate for well-being to help clients maintain quality of life.

The ideal candidate is empathetic, well-organized, and capable of collaborating with families, healthcare providers, and community organizations to deliver person-centered care.

Qualifications

• Bachelor's degree preferred in Social Work, Human Services, psychology, or related field.
• High school diploma required.
• Experience in case management, social services, homecare, or working with older adults preferred.
• Experience supporting trauma survivors, seniors, or vulnerable populations highly desirable.
• Excellent interpersonal, written, and verbal communication skills.
• Strong organizational skills to manage multiple cases and priorities.
• Knowledge of community resources and healthcare systems.
• Proficiency with MS Word, Excel, and electronic documentation systems.
• Bilingual Yiddish and/or Russian preferred.

Responsibilities

• Manage an assigned caseload of Homecare clients.
• Conduct comprehensive home visits to assess clients' needs.
• Identify unmet needs including healthcare, mental health, financial assistance, social support, and QoL services.
• Develop individualized care plans based on client circumstances and goals.
• Maintain regular communication with clients and families via phone and in-person visits.
• Coordinate services with healthcare providers and community agencies.
• Assist clients in accessing homecare services, counseling, financial assistance, and resources.
• Advocate for clients to ensure compassionate, respectful care.
• Maintain accurate, confidential case documentation per guidelines.
• Prepare reports summarizing client progress and ongoing needs.
• Participate in staff meetings and professional development.
• Perform additional case management and administrative duties.
