Job summary

Sbhonline in New York seeks a compassionate Holocaust Survivor Homecare Case Worker to assess needs, coordinate services, advocate for clients, and manage caseloads of seniors in the New York area. Holocaust Survivor Homecare Case Worker

Position Overview

We are seeking a compassionate and organized Homecare Case Worker to provide comprehensive support to Holocaust Survivors. This role involves assessing client needs, coordinating services, advocating for clients, and helping improve their overall quality of life through ongoing case management and community resource coordination.

Key Responsibilities

• Manage a caseload of Homecare clients.
• Conduct in-home assessments to evaluate clients' physical, emotional, social, and financial needs.
• Identify unmet needs beyond homecare services, including healthcare, mental health, social support, and quality‑of‑life concerns.
• Develop individualized care plans based on each client's circumstances and goals.
• Maintain regular communication with clients and their families through home visits and follow‑up calls.
• Coordinate with healthcare providers, community organizations, and service agencies to ensure clients receive appropriate care and resources.
• Assist clients in accessing homecare, counseling, financial assistance, and other supportive services.
• Advocate for clients to ensure they receive respectful, compassionate, and comprehensive care.
• Maintain accurate and timely case notes and documentation in compliance with agency and funding requirements.
• Prepare reports on client status, service delivery, and ongoing needs.
• Attend staff meetings, trainings, and perform other duties as assigned.

Qualifications

• High school diploma required; bachelor's degree in Social Work or a related field preferred, or equivalent experience in social services or case management.
• Experience working with older adults, trauma survivors, or other vulnerable populations is strongly preferred.
• Excellent interpersonal, communication, and organizational skills.
• Ability to manage multiple cases while maintaining accurate documentation.
• Strong time management skills and the ability to work independently and collaboratively.
• Knowledge of community resources, healthcare systems, and social service programs.
• Proficiency in Microsoft Office (Word, Excel) and data entry systems.
• Bilingual Yiddish and/or Russian is preferred.
