Job summary

Sbhonline is seeking a compassionate Case Worker to provide comprehensive Homecare program services for Holocaust Survivors in Brooklyn, NY. You will assess client needs, coordinate services, advocate for well-being, and support quality of life through collaboration with families, healthcare providers, and community partners.

The role requires empathy, strong organization, and a dedication to serving older adults and vulnerable populations with a focus on advocacy and individualized care

Qualifications

  • High school diploma required.
  • Bachelor's degree in Social Work (BSW), Human Services, Psychology, or related field preferred.
  • Experience in case management or social services preferred.
  • Experience with trauma survivors, seniors, or vulnerable populations desirable.
  • Excellent interpersonal, written, and verbal communication skills.
  • Strong organizational skills and ability to manage multiple cases.
  • Ability to work independently and with multidisciplinary teams.
  • Knowledge of community resources and healthcare systems.
  • Proficiency with MS Word/Excel and electronic records.

Responsibilities

  • Manage an assigned caseload of Homecare clients.
  • Conduct comprehensive home visits to assess clients' needs across physical, emotional, social, and environmental domains.
  • Identify unmet needs including healthcare, mental health, financial assistance, social support, and quality-of-life services.
  • Develop individualized care plans based on client circumstances and goals.
  • Maintain regular communication with clients and families via phone and in-person visits.
  • Coordinate services with healthcare providers, community agencies, and support organizations.
  • Assist clients in accessing homecare services, counseling, financial assistance, and community resources.
  • Advocate for clients to ensure compassionate, respectful, and comprehensive care.
  • Maintain accurate, timely, confidential case documentation per agency/regulatory requirements.
  • Prepare reports on client progress, service delivery, and ongoing needs.
  • Participate in staff meetings, case conferences, and professional development.
  • Perform additional case management and administrative duties as needed.

Position Overview

We are seeking a compassionate and dedicated Case Worker to provide comprehensive support services to Holocaust Survivors enrolled in our Homecare Program. This role is focused on assessing client needs, coordinating services, advocating for client well-being, and helping individuals maintain the highest possible quality of life.

The ideal candidate is empathetic, highly organized, and passionate about serving older adults and vulnerable populations while collaborating with families, healthcare providers, and community organizations.

Qualifications

  • Bachelor's degree in Social Work (BSW), Human Services, Psychology, or a related field is preferred.
  • Previous experience in case management, social services, homecare, or working with older adults is preferred.
  • Experience supporting trauma survivors, seniors, or other vulnerable populations is highly desirable.
  • Excellent interpersonal, written, and verbal communication skills.
  • Strong organizational skills with the ability to manage multiple cases and priorities.
  • Ability to work independently while collaborating effectively with multidisciplinary teams.
  • Knowledge of community resources, healthcare systems, and social service programs.
  • Proficiency in Microsoft Word, Excel, and electronic documentation systems.
  • Bilingual Yiddish and/or Russian is preferred.
