Freeman Health System's Ozark Center seeks a Community Support Specialist to perform case management for CSTAR clients, guiding them toward recovery and community integration through trauma-informed care and collaborative planning.

You will participate in assessments, develop holistic treatment plans, coordinate services, and advocate for clients while supporting life skills training and resource access within the agency and community.

Qualifications

QAP or QMHP certification required
Bachelor's degree in human services from an accredited institution
Four-year degree with qualifying experience (or any four-year combination)
AAS in Behavioral Health Support is acceptable
At least four years of qualifying experience

Responsabilités

Participate in initial assessment and service plan development for CSTAR individuals.
Develop a holistic treatment plan addressing physical, mental, emotional and spiritual needs.
Conduct periodic reassessments to evaluate alterations to the Treatment plan.
Arrange and refer for services and resources within the agency and community.
Participate in the inter-disciplinary treatment team to ensure continuity of services.
Provide training in life skills and resource acquisition.
Advocate on behalf of clients to obtain needed services.
Monitor service delivery and follow up to ensure appropriate therapy.
