Job summary

Qualifications

• Requires valid Driver's License with good driving record and auto liability insurance as per Texas statute.
• Minimum High School or equivalent and one year experience in social services.
• Knowledge of substance abuse and HIV.
• Knowledge of available community resources.

Responsibilities

• Provide client-centered case management linking clients to health care, psychosocial and supportive services.
• Assess client needs, develop individualized service plans, coordinate services, and periodically reevaluate plans.
• Advocate for clients and refer to external services as needed.
• Input data into databases accurately and timely.
• Prepare reports and monthly programmatic outputs for supervisor.
• Conduct outreach and community education in a culturally sensitive manner.
• Ensure confidentiality and professional standards in all interactions.
