Qualifications

Responsibilities

• Performs initial and ongoing assessment of patient and family. Documents ongoing assessment per TriHealth guidelines.
• Plans care for patient and family based on assessment, develops goals, discharge planning, and communicates plan of care to others.
• Implements plan of care, provides safe care, maintains patient autonomy and dignity, and documents interventions.
• Evaluates the plan of care with patient/family and health care team, revising as needed.
• Performs technical skills and safely administers medications/treatments, monitors effects.
• Demonstrates leadership and supports learning experiences for students and collaborates with the care team.
