Joseph P Addabbo Family Health Center Inc. is seeking a Prevention Educator for the Primary Care HIV Prevention (PCHP) program. The role focuses on education, HIV testing, PrEP/PEP counseling, and linking clients to care within JPAFHC and partner communities.

You will work with patients, staff, and community members to host outreach events, provide on-site and off-site testing, and develop educational materials to promote sexual health and HIV prevention.

Qualificações

• Associate’s or Bachelor’s Degree in Health Education, Public Health, Social Work, or related field preferred; two years’ related experience accepted.
• LGBTQIA+ community encouraged to apply.
• Driver’s license and personal vehicle preferred or able to use public transportation.
• At least 2 years HIV/AIDS prevention, outreach, and health promotion experience preferred.
• Experience with PLWHA, LGBTQIA+, and communities of color preferred.
• Proficient in MS Word, Excel, PowerPoint; EMR experience preferred.
• Bilingual English/Spanish preferred.

Responsabilidades

• Engage patients and community in HIV education, screening, and sexual health promotions.
• Conduct point-of-care visits to discuss PrEP/PEP and condom usage.
• Outreach and recruitment at seminars, events, and tabling in service areas.
• Establish partnerships with local schools, NYCHA, and community organizations.
• Build relationships with non-traditional partners to provide education and outreach.
• Perform HIV rapid testing and connect clients to care using status-neutral approach.
• Engage individuals in harm reduction strategies including PrEP/PEP and education.
• Maintain records and track outreach performance.
• Assist in creating educational materials for patients and staff.
• Develop and execute patient education activities for HIV awareness days.
• Prepare and present educational materials at internal and public events.
• Reduce barriers to care by coordinating with providers and staff.
• Participate in staff meetings and trainings.
• Perform special projects as assigned.
