Job summary

Wyckoff Heights Medical Center is seeking a program-focused professional to deliver and facilitate access to comprehensive sexual and behavioral health services for clients at risk for HIV/STI/HCV. You will conduct HIV testing, counseling, and outreach, recruit clients in the community, and assist with scheduling medical appointments for prevention and treatment services.

Qualifications

• Associate Degree or certification equivalent required.
• Minimum of 3 years job-related experience.
• Knowledge of HIV and sexual health services; outreach and education experience.
• Active certification in HIV testing and counseling preferred.
• Experience in HIV prevention, counseling, testing and human services is desired.
• Strong verbal and written communication skills.

Responsibilities

• Deliver and facilitate access to comprehensive sexual and behavioral health services for clients at risk for HIV/STI/HCV.
• Conduct HIV testing and counseling with clients.
• Perform program outreach to recruit clients and disseminate information in the community.
• Assist clients in scheduling medical appointments for sexual health services at Wyckoff.
• Facilitate linkage to PrEP/PEP, housing, benefits enrollment, and other social services.
• Provide risk-reduction education and counseling; promote materials for prevention programs.
• Maintain timely client data in documentation templates and databases; keep charts up-to-date.
• Attend supervision and trainings; participate in program meetings and requirements.
• Promote and develop educational materials for prevention programs.
