Job summary

Center for Family and Child Enrichment in Florida is seeking an Outreach Worker for the SHAPP/HIV Program. Under general supervision, you will engage communities, provide health education, and link clients to HIV prevention and care services.

Responsibilities include field outreach, risk reduction counseling, testing referrals, and partner coordination. A High School diploma is required; an Associate degree is preferred; HIV/AIDS 500/501 certification is a plus.

Qualifications

• High School Diploma or GED required.
• Associate degree preferred in Public Health, Human Services, Social Work, or related field.
• HIV/AIDS 500/501 Certification preferred or willingness to obtain within designated timeframe.

Responsibilities

• Conduct HIV/STI prevention outreach, education, and linkage services to individuals, families, and communities through field outreach and events.
• Provide sexual health education, risk-reduction counseling, and prevention resources related to HIV and other STIs.
• Promote HIV testing, PrEP/PEP awareness, and access to prevention and treatment services.
• Engage priority populations including LGBTQ+ communities and underserved groups.
• Assist with HIV testing referrals and linkage to care with follow-up as needed.
• Coordinate with CFCE staff and partners to ensure continuity of care and service coordination.
• Enter accurate client data into EHR and program databases; maintain confidentiality per HIPAA.
