Job summary

The Georgia Department of Public Health HIV Prevention Program seeks a qualified professional to provide HIV prevention, testing, counseling, outreach, and linkage-to-care services across the district. The role connects HIV clients to medical care and supports access to PrEP and prevention services with ARTAS implementation.

Responsibilities include outreach, testing, data monitoring, and quality improvement activities, with collaboration across public health programs and partners.

Qualifications

• Bachelor's degree required in education, social, behavioral, or biological sciences or a related field.
• Experience in HIV prevention, HIV care, sexual health, infectious disease, or related public health program.
• Experience providing community outreach, health education, counseling, case management or linkage-to-care services.

Responsibilities

• Link HIV-positive individuals to care, including perinatal care coordination.
• Provide community outreach, point-of-care testing and counseling services as needed.
• Provide referrals and linkage into PrEP program and partner services.
• Assist with strategies to identify individuals living with HIV not engaged in care and re-engage them.
• Collaborate with other public health programs and workgroups.
• Implement quality assurance and quality improvement projects.
• Prepare and submit monthly service reports and quarterly narratives.
• Other duties as assigned.
