Job summary

State of Georgia is seeking a dedicated SS Prg. Coordinator 2 - EHE Linkage Coordinator in Cobb and Douglas Counties. The role focuses on HIV testing, linkage to care, retention, and re-engagement in medical care, with grant funding and state-wide health program support.

Qualifications

• Bachelor’s degree in a related field from an accredited college or university and two years of job-related experience.
• Or two years of experience at the lower level Social Svcs Prgm Coord 1 (SSP030) or equivalent.
• Excellent verbal and written communication skills. Valid Georgia Driver’s license with a good driving history.

Responsibilities

• Implement the ARTAS strategy for individuals newly diagnosed with HIV or lost to care.
• Coordinate timely linkage, referrals, and follow-up for HIV care, partner services, STI and hepatitis services, TB screening, mental health, substance use treatment, and other supportive services as needed.
• Promote routine and opt-out HIV testing, HIV self-testing, and other HIV prevention and testing strategies in healthcare and community settings.
