Qualifications

• Unrestricted medical license in the state of practice.
• Board‑certified in Internal Medicine or Family Practice.
• Experienced in population health management.
• Strong preference for experience with complex patients.

Responsabilités

• Provide outpatient care for patients with chronic infectious diseases.
• Manage chronic conditions through collaboration with healthcare teams.
• Educate patients about their health and treatment options.
• Develop transitional care plans tracking patient journeys.
