Job summary

AbsoluteCare is seeking a physician to provide outpatient HIV primary care within a large, integrated care team. You will diagnose and manage HIV and related infectious diseases, develop care plans, educate patients, and coordinate with specialists to meet each patient’s needs.

The role emphasizes evidence-based medicine in the PCMH model, collaboration with social work, care management, and behavioral health.

Qualifications

• Unrestricted Medical License in the state of practice.
• Board certified Internal Medicine or Family Practice.
• AAHIVM certification or Board Certification in Infectious Diseases.
• BLS certification.
• DEA certification.
• Experience in population health management preferred.
• Experience with psychosocially and medically complex patients preferred.
• Knowledge of NCQA PCMH model preferred.
• Must work well in a multidisciplinary team with collaboration.
• Proficient in analyzing and leveraging EMR reporting.

Responsibilities

• Provide care for people living with HIV and related chronic infectious diseases.
• Provide outpatient care consultation and ongoing treatment of chronic infectious diseases.
• Deliver comprehensive care for patient panels, coordinating with primary care and specialists.
• Work in a team environment with social work, care management, behavioral health, pharmacy, nutrition, and more.
• Implement care plans in collaboration with patients, families and team members.
• Develop transitional care plans and track paneled patients across hospitalization, ER, SNFs and home care.
• Prevent ER visits, hospital admissions and readmissions through vigilant monitoring.
• Educate patients about chronic diseases and healthy choices; discuss advanced directives.
• Practice evidence-based medicine within the PCMH model of care.
• Lead coordination of care for HIV/AIDS patients with an emphasis on teamwork.
