Job summary

AbsoluteCare Medical Center & Pharmacy in Chicago, IL seeks a physician to provide HIV and chronic infectious disease care in the outpatient setting, developing diagnostic and treatment plans and educating patients about their illnesses.

You will work within a large, integrated care team, coordinating with social work, care management, pharmacy, nutrition, and other specialists to deliver whole-person care. This role emphasizes PCMH-based care and proactive management to prevent hospital visits.

Qualifications

• Unrestricted Medical License in the state of practice.
• Board certified Internal Medicine or Family Practice.
• Certified by AAHIVM OR Board Certification in Infectious Diseases.
• BLS.
• DEA.
• Experience in population health management is preferred.
• Experience with psychosocially and medically complex patients is preferred.
• Knowledge or experience with NCQA PCMH model is preferred.
• Ability to work in a multidisciplinary team with cross-provider collaboration.
• Innovative self-starter with problem-solving mindset.
• Experience maximizing EMR reporting capabilities.
• Strong interpersonal and communication skills.
• Proficient in Microsoft Office Suite.

Responsibilities

• Provide care for people living with HIV and associated conditions in outpatient settings.
• Develop diagnostic and treatment care plans and educate patients.
• Deliver comprehensive care for a patient panel in collaboration with primary care and specialists.
• Work in a team environment with social work, care management, pharmacy, nutrition, and other staff.
• Implement plans of care with patients and families and other team members.
• Develop transitional care plans across hospital discharges and home care.
• Monitor disease processes to avoid ER visits and readmissions.
• Educate patients on chronic diseases and healthy choices.
• Practice evidenced-based medicine aligned with the PCMH model.
• Coordinate care for HIV/AIDS patients and lead multidisciplinary teams.
