Job summary

St. Luke's University Health Network is seeking a Medical Case Manager to provide comprehensive case management for clients with HIV/AIDS under the HOPE Program. You will coordinate services, advocate for clients, and ensure confidentiality in line with funding requirements.

The role requires collaborating with physicians, nurses, and social work colleagues, maintaining accurate records, and participating in ongoing training to enhance care delivery. Bilingual Spanish skills are preferred.

Qualifications

• Bachelor’s degree in social work, psychology, or sociology or related field.
• A minimum of one year experience in HIV/AIDS management and assessment techniques preferred.
• Bilingual capability—Spanish language preferred.

Responsibilities

• Provide case management services to persons with HIV/AIDS.
• Intake, screening, assessment, and development of service coordination plans.
• Referrals for services and placement; monitor plan effectiveness and outcomes.
