Job summary

Erie Family Health Centers in Chicago seeks a Lead Medical Case Manager for the Lending Hands for Life (LHL) program. This full-time role provides supervision and hands-on HIV case management for high-need clients within our center network.

Responsibilities include supervising staff, handling HIV intake, coordinating treatment adherence, benefits enrollment, and referrals. Strong BA/BS required with related experience; bilingual skills are a plus in serving a diverse patient population.

Qualifications

• 3+ years in case management or similar role; 1–2 years supervisorial experience preferred.
• CPR Certification required.
• Familiarity with issues affecting individuals living with HIV.
• Verbal and written communication skills.
• Intermediate proficiency in Microsoft Office preferred.
• Bilingual English/Spanish a plus; 50% of LHL patients prefer Spanish.

Responsibilities

• Provide 1-on-1 supervision, leadership, coaching, and support to direct reports.
• Manage HIV intake, HIV pool in EMR, and assign cases to team members.
• Oversee staff schedules, time-off requests, and hiring/onboarding/training.
• Monitor workflows and performance metrics; ensure program goals are met.
• Ensure quality assurance of case management documentation; provide guidance.
• Communicate new initiatives and program changes to the LHL Program Manager.
