Job summary

Erie Family Health Centers is seeking a LHL Medical Case Manager Team Leader in Chicago, IL to provide leadership, supervision, and medical case management for people living with HIV. You will guide the HIV team, coordinate care, and ensure high-quality service delivery.

Responsibilities include supervising staff, managing HIV intake, collaborating with medical providers and community partners, and advancing program goals.

Qualifications

• CPR Certification required.
• Familiarity with issues affecting individuals living with HIV.
• Verbal and written communication skills.
• Intermediate proficiency in Microsoft Office preferred.
• Bilingual fluency (English/Spanish) a plus.

Responsibilities

• Team Leader Supervision: provide 1-on-1 supervision, leadership, coaching, and support to direct reports; manage HIV intake and EMR workflow; oversee staffing and training; monitor performance metrics; ensure quality documentation.
• Case Management and Program Support: manage higher-need HIV+ clients; coordinate medical treatments, benefits enrollment, and referrals; document care in EMR and state database; participate in quality improvement; assist with events and trainings.
