Qualifications

• Requires an Associate's Degree in health care or human services related field.
• Minimum of 2 years in a health care setting.
• Proficiency with Microsoft Office and data entry.
• American Heart Association Basic Life Support - State of Pennsylvania within 90 days.

Responsibilities

• Provides case management clerical support including tracking and assistance with provision of needed documentation for recertification and service provision.
• Assists clients in preparing applications and other paperwork for insurance and other entitlement programs.
• Updates patient information into CAREWare, EPIC and the Case management services data base.
• Schedules transportation for medical appointments, procures gift cards and other case management items and assures accurate tracking of these items based on internal audit recommendations.
• Assists with case management chart audits and quality improvement initiatives as necessary.
