Job summary

Lehigh Valley Health Network, Allentown, PA seeks an Associate HIV Case Manager to provide clerical support to HIV case managers, help with documentation, and assist clients with insurance and entitlement paperwork.

You will update systems (CAREWare, EPIC), arrange transportation, track items, and participate in chart audits and quality improvement initiatives; associates degree required, 2 years in healthcare, BLS certification within 90 days.

Qualifications

• Associate’s Degree in healthcare or human services related field.
• 2 years in healthcare setting.
• Microsoft Office and data entry.
• American Heart Association Basic Life Support - State of Pennsylvania within 90 Days.

Responsibilities

• Provides case management clerical support including tracking and assistance with provision of needed documentation for recertification and service provision.
• Assists clients in preparing applications and other paperwork for insurance and other entitlement programs.
• Updates patient information into CAREWare, EPIC and the Case management services data base.
• Schedules transportation for medical appointments, procures gift cards and other case management items and assures accurate tracking of these items based on internal audit recommendations.
• Assists with case management chart audits and quality improvement initiatives as necessary.
