Responsibilities

• Provide case management services to persons with HIV/AIDS.
• Intake and screening, assessment, service coordination plans, referrals, monitoring and evaluations.
• Adhere to confidentiality regulations and documentation standards.
• Consult with health care team to facilitate interdisciplinary care.
• Attend mandatory and additional training as required.
• Support translation for non-English speaking clients as needed.
• Engage in client outreach and maintain accurate records.
