The role includes intake, assessment, service coordination, referrals, and ongoing monitoring to ensure effective care and adherence to confidentiality regulations. The position requires collaboration with physicians, nurses, and other professionals, with a focus on culturally sensitive engagement of diverse client populations.

Qualifications

• Bachelor's degree in social work, psychology, or sociology or related field.
• A minimum of one year experience in HIV/AIDS management and assessment techniques preferred.
• Bilingual capability—Spanish language preferred.

Responsibilities

• Provide case management services to persons with HIV/AIDS.
• Intake and screening, assessment, preparation of service coordination plans, referrals, monitoring, advocacy, and outcome evaluations.
• Consult with physicians, nurses, therapists and other members of the health care team to facilitate interdisciplinary care.
• Adhere to confidentiality regulations regarding HIV/AIDS care and services.
• Attend mandatory case management training and orientation; participate in additional training as available.
• Maintain accurate client records and documentation according to policy and funding contracts.
• Coordinate and facilitate support groups or agency programs; translate services as needed for non-English speakers.
• Represent the agency in public relations and fundraising events; serve as a professional role model.
