Wyckoff Heights Medical Center in Brooklyn, NY is seeking an outpatient clinical nurse to oversee training for clinical staff in Partnership for Health (PFH) interventions, support integration into sessions, monitor fidelity, track service delivery, and report data.

The role requires an Associate Degree or equivalent and NY LPN/RN with HIV Specialist certification; 1–3 years HIV experience preferred. Mon-Fri, non-union, with emphasis on coordinating patient flow and adherence support.

Formación

• Associate degree or equivalent required; minimum 3 years' related experience.
• 1–3 years HIV-related experience preferred; NY LPN or RN with HIV Specialist cert.
• Proficient with computers, EMR, and Microsoft Office.

Responsabilidades

• Oversee training for clinical staff in Partnership for Health (PFH) interventions and support integration into clinical sessions.
• Monitor fidelity of PFH delivery and track service delivery data.
• Coordinate booster trainings and reporting of data.
• Enter and track patient data for PFH participants.
• Provide nursing support to optimize patient flow and care in the clinic.
• Lead daily huddles with the health care team and liaise with providers.
