Qualifications

• Bachelor's Degree in Social Work or related field required.
• Experience and knowledge of HIV, entitlement programs, and community resources preferred.
• Strong communication, organizational, documentation, counseling and follow-up skills are important.

Responsibilities

• Provide medical case management services to patients per standards and procedures.
• Offer risk-reduction counseling, adherence counseling, and education on HIV care.
• Assess patient needs and link to healthcare, social services, and community resources.
• Maintain HIPAA compliance and complete timely documentation and follow-up.
