Job summary

Circle Health Services is seeking a dedicated Case Manager to provide direct services to clients enrolled in the Early Intervention Program. Responsibilities include case management, development of care plans, and coordinating with medical staff and transportation services to support HIV-related care.

Ideal candidates hold a social work degree with LSW/LISW licensure and have 2+ years of related experience, including work with HIV+ populations.

Qualifications

• Professional II Case Manager – bachelor’s degree in social work and LSW.
• Master’s degree in social work and LISW or equivalent preferred.
• Licensure in good standing in Ohio.
• Two+ years of related experience with HIV+ population.
• Willingness to engage with Ryan White program and Circle Health Services protocols.

Responsibilities

• Provide case management and advocacy for client caseloads.
• Develop and review case plans; conduct conferences.
• Maintain client records and report service statistics.
• Coordinate treatment with clinical staff; liaise with transportation and Linkage to Care.
• Assist with intake processes for Early Intervention Program.
