Job summary

SisterReach is seeking a full‑time Community Care Connector in Memphis. You will perform outreach, form re‑engagement plans, and coordinate with clinical partners to support people living with HIV in reconnecting to care.

The role emphasizes trauma‑informed, harm‑reduction, and culturally responsive approaches to assist clients across community settings, including in‑person visits and mobile outreach.

Qualifications

• Experience in community health or HIV services is preferred.
• Ability to engage clients who face stigma, poverty, or housing instability.
• Knowledge of social determinants of health and care navigation.

Responsibilities

• Conduct direct multi‑channel outreach to individuals identified through referral pathways.
• Co‑develop individualized re‑engagement plans with participants.
• Coordinate warm handoffs to clinical partners and assist with appointment scheduling.
• Provide non‑clinical wrap‑around support including housing, food, transportation, and mental health resources.
• Maintain accurate program data and participate in trainings and meetings.
