Qualifications

Responsibilities

• Conduct HIV testing, outreach, and linkage to care in clinical settings.
• Recruit and enroll clients; promote ARV education and PrEP services.
• Coordinate care, perform education and counseling, and maintain records.
• Attend case conferences; implement service plans and action steps.
• Document encounters and services in EMR and CAREWare as prescribed.
• Adhere to applicable laws, policies, and quality improvement processes.
