Aufgaben

• Provide non-medical support for PLWH, including peer counseling and outreach.
• Locate and re-engage PLWH lost to care and address barriers.
• Facilitate linkage to care with open communication with PLWH.
• Maintain contact with high-risk clients to prevent dropout from care.
• Collaborate to provide HIV/STD education and linkage to services.
• Input data and maintain Ryan White databases for reporting.
