Job summary

Prisma Community Care in Phoenix, AZ is seeking a Ryan White Case Manager to provide case management for clients living with HIV. You will assess needs, develop service plans, and connect clients to medical and social services while supporting adherence and viral suppression.

The role emphasizes cultural competency, documentation, insurance navigation, and collaboration with internal and external partners to empower clients to self-manage their care.

Qualifications

• Bachelor’s degree in social work or related field, or four years of relevant experience with a high school diploma.
• Ability to communicate effectively with culturally diverse populations.
• Knowledge of HIV, housing, health insurance and barriers to care.
• Understanding of gender identity and LGBT-related concerns.
• Proficient with Microsoft Office and EMR systems, HIPAA compliance.
• Strong organizational and multitasking abilities.
• Excellent written and verbal communication; compassionate and professional.
• Commitment to diversity and inclusion.

Responsibilities

• Conduct psychosocial assessments and assess eligibility for Ryan White services.
• Create individualized service plans to help clients access and maintain care.
• Close gaps in care and support adherence to treatment and visits.
• Manage an active caseload with billable activity targets for Ryan White program.
• Document interactions in EMR and state reporting systems within timelines.
• Provide information and referrals to external partners for additional support.
• Navigate insurance and assist with patient assistance programs.
• Collaborate with teams and leadership to improve services and processes.
