El Proyecto is seeking a licensed vocational nurse to coordinate HIV care within a multidisciplinary team. You will assess patient needs, develop care plans, and document in the EMR while ensuring quality and confidentiality.

Responsibilities include re-engaging out-of-care patients, coordinating with providers, and supporting education on treatment and adherence. CA LVN license and bilingual ability are preferred.

Competenze

• LPN license in the State of California.
• High school diploma or equivalent.
• Minimum 3 years of related experience.
• Bilingual (English/Spanish) preferred.
• Proven ability to work with HIV care teams.
