Responsibilities

Coordinate care with interdisciplinary team, patient, family, and others to provide effective HIV/AIDS care.
Formulate and modify care plans based on assessments and treatment changes.
Identify unmet biomedical and non-medical needs and coordinate with providers to address gaps.
Document in EMR and prepare reports for funding sources.
Engage patients to re-engage in treatment and maintain continuity of care.
Participate in community events and meetings.
