Job summary

El-Proyecto-Del-Barrio in Los Angeles is seeking a temporary Care Coordination Assistant to support the Medical Care Coordination Program, helping patients access services, assess needs, and navigate health decisions.

You will liaise with HIV testing sites and clinics, update care plans, track outcomes, and ensure compliance with regulatory standards while supporting diverse patients.

Qualifications

Bachelor's degree in nursing, social work, related field or equivalent experience.
0 to 1 year of clinical experience.
Bilingual English/Spanish preferred.

Responsibilities

Identify, assist, and connect eligible patients to services and resources.
Coordinate linkage and referrals to services.
Update care plans, follow up with clients, enter patient data, and track outcomes.
Liaise between HIV counseling and testing sites and the clinic to enroll new clients in medical care.
Coordinate services with specialty clinics and referrals.
Interact professionally with patients and their families.
Consider patient cultural, psychological, social, and environmental factors.
Ensure regulatory compliance and organizational standards.
Perform other duties as assigned.
