Job summary
San Francisco Community Health Center seeks a Licensed Vocational Nurse (LVN) or Licensed Psych Tech (LPT) in San Francisco, CA to provide field-based nursing and case management for HIV-positive clients in Intensive Case Management (ICM) or Enhanced Care Management (ECM) programs. The LVN/LPT is the central medical provider on a mobile team and supports assessments, care plans, and medication adherence.

Qualifications
- Candidate must be a Licensed Vocational Nurse or Licensed Psych Tech in the state of California.
- Knowledge of HIV/AIDS, STIs, substance use, and mental health issues.
- Experience working with LGBTQ populations.
- Familiarity with HIV/AIDS issues in communities of color.
- Team based care and harm reduction philosophy.
Responsibilities
- Provides mobile nursing care and case management to HIV positive clients in ICM/ECM programs.
- Supports field-based health and psychiatric assessments and monitoring.
- Offers enhanced medication support including dispensing, DOT, and IM Depo injections.
- Coordinates with pharmacies and develops individualized nursing care plans.
- Documents in the electronic medical record (Epic) and maintains timely reports.
