We are currently seeking a Care Coordinator in our 400 Broadway location.
Lead a team of patient navigators and patient assistants to ensure care coordination services are provided to all clients assigned to meet clients’ needs for better health outcomes. Provide day‑to‑day supervision to the patient navigators and patient assistants assigned by weekly schedule monitoring, chart review, face‑to‑face case discussion, and performance reviews. Responsible for implementation of service/care plan, supported by patient navigators. Conduct outreach and recruitment for Apicha CHC to promote the Agency services.
BA/BS in related field with two (2) years of qualifying experience required. An advanced degree in one of the qualifying fields may be substituted for one (1) year of qualifying experience; experience working with target populations strongly preferred. Target populations: HIV/AIDS who are virally unsuppressed, individuals with serious mental illness, serious substance use disorders (SUDs), and/or chronic hepatitis. Ability to adapt to a changing environment. Appreciation of and commitment to Apicha’s mission and values required. Excellent MS Office skills (Outlook, Word, Excel, PowerPoint) required. Ability to work effectively in both a collaborative as well as individual setting. Must possess ability to interact with all levels of external and internal customers with a high level of professionalism and tact in representing Apicha CHC. Excellent customer service skills to assist in resolving member complaints. Strong verbal and written communication skills – for both internal and external audiences.
