We are currently seeking a Care Coordinator in our 400 Broadway location.

Position Summary

Lead a team of patient navigators and patient assistants to ensure care coordination services are provided to all clients. Provide day‑to‑day supervision to the patient navigators and patient assistants, including weekly schedule monitoring, chart review, face‑to‑face case discussion, and performance reviews. Responsible for implementation of service/care plans, supported by Patient Navigators. Conduct outreach and recruitment for Apicha CHC to promote agency services.

Qualifications

• BA/BS in a related field with two (2) years of qualifying experience required.
• Advanced degree in one of the qualifying fields may substitute for one (1) year of qualifying experience.
• Experience working with target populations: HIV/AIDS who are virally unsuppressed, individuals with serious mental illness, serious substance use disorders, and/or chronic hepatitis.
• Ability to adapt to a changing environment.
• Commitment to Apicha’s mission and values.
• Excellent MS Office skills (Outlook, Word, Excel, PowerPoint).
• Ability to work effectively in both collaborative and individual settings.
• Professional interaction with all levels of external and internal customers with care and tact.
