Qualificações

Responsabilidades

• Provide continuum of care: intake, referral, assessment, coordination, monitoring, follow-up.
• Develop comprehensive individualized care plans.
• Monitor and re-evaluate care plans every 6 months and adapt as needed.
• Coordinate services through Care Team Meetings.
• Enter and maintain confidential client documentation.
• Conduct home visits and psychosocial groups as appropriate.
• Maintain relationships with partner organizations to support client needs.
• Participate in program committees and other duties as directed by leadership.
