Hranbct in New Britain, CT seeks a Medical Case Manager to provide client-centered care and implement treatment plans for HIV/AIDS services. The role focuses on linking clients to medical, mental, and dental care, coordinating services, and documenting progress.

Primary duties include intake, care plan development, ongoing monitoring, and regular reassessments every six months, with conduct of care conferences and home visits as needed. Bilingual abilities preferred.

Competenze

• Association degree in health or human services or equivalent.
• Ability to work with culturally diverse populations.
• Knowledge of HIV/AIDS, health disparities and related issues.

Mansioni

• Provide continuum of care services including intake, referral, assessment, coordination, monitoring and follow-up.
• Develop comprehensive, individualized care plans and reassess them at least every 6 months.
• Coordinate services to meet medical, mental and dental care needs.
• Enter client data into funder-determined software and maintain confidential records.
• Communicate with other case managers and participate in team meetings and conferences.
• Conduct home visits and psychosocial groups as appropriate.
• Develop relationships with partner organizations to assist with housing, mental health, finances and advocacy.
