Qualifications

• Bachelor's Degree in Social Work or a Social Work-related field is required.
• Strong knowledge of case management standards and HIPAA requirements.
• Bi-lingual Spanish language skills strongly preferred.

Responsibilities

• Provide medical case management services to patients in accordance with established standards and departmental procedures.
• Conduct risk-reduction counseling and adherence counseling to promote positive health outcomes.
• Educate patients about HIV care, treatment adherence, and available community resources.
• Coordinate referrals to healthcare, social services, and entitlement programs.
• Maintain accurate documentation and ensure timely follow-up of services.
