Job summary

DAP Health Sunrise in Palm Springs, CA seeks an Early Intervention Services (EIS) Specialist to connect HIV/HCV-positive clients to medical care quickly. The role involves counseling, risk reduction education, and coordinating referrals within DAP Health clinics and community partners.

You will assess social determinants of health, educate clients and partners on HIV prevention including PrEP/PEP, and help navigate insurance and lab processes.

Qualifications

• Bachelor's degree or experience in medical or health-related fields (social work, counseling, nursing, public health, or health education).
• Current American Heart Association or American Red Cross BLS certification.

Responsibilities

• Deliver HIV and HCV/STI education and counseling to clients (newly diagnosed or re-engaging in care).
• Assess social determinants of health to tailor prevention strategies for infectious and chronic illnesses.
• Educate clients and partners on HIV prevention methods and facilitate PrEP/PEP referrals.
• Facilitate access to care and remove barriers for clients newly diagnosed with HIV or out of care.
• Maintain accurate documentation and update Electronic Health Records (HCC, Epic).
• Meet clients on a walk-in basis to provide support and address immediate needs.
