Job summary

Equitas Health is seeking a Benefits Navigator to assist clients in determining eligibility and enrolling into OHDAP and Ryan White programs, while linking them to Medicaid, Medicare, and Federal Marketplace plans across Ohio.

Qualifications

• Associate degree with at least one year relevant experience or two years with coursework.
• Bachelor’s degree in human services or related field preferred.
• Valid Ohio driver’s license and reliable transportation.
• Experience in health navigation or social services is a plus.

Responsibilities

• Provide navigation and support individuals living with HIV so that they may be retained in HIV medical care and maintain viral suppression.
• Provide referrals to HIV screening programs and care coordinators.
• Collaborate with community partners to connect clients to insurance options.
• Screen for insurance options and assist eligible clients to apply for Medicaid, Medicare and marketplace plans.
• Explain plan choices, costs, and consumer rights under various options.
• Document applications and maintain timely submissions.
• Demonstrate NASW-guided professional standards and ethics.
