The position supports travel to meet clients, utilize data systems, and maintain documentation within required timelines, aiming for effective collaboration with healthcare teams and community partners.

Qualifications

• Minimum of BS/BSW and LSW required.
• Must have sensitivity to HIV/AIDS, minority health, and LGBTQ+ community.
• Ability to work in a community-based setting and coordinate with multiple agencies.

Responsibilities

• Provide high quality case management to access medical services, Ryan White benefits, and housing resources.
• Conduct comprehensive psychosocial assessments and biannual updates for clients with HIV/AIDS.
• Develop and monitor individualized case plans and coordinate with community services.
