Job summary

Equitas Health is seeking a Medical Social Worker to provide comprehensive case management for HIV+ clients across Ohio. The role requires an LSW and involves coordinating access to medical care, insurance, and support services while maintaining confidentiality across systems.

Travel is involved to meet clients; the worker will conduct psychosocial assessments, develop care plans, and connect clients with Ryan White programs, housing support, and transportation resources as needed.

Qualifications

• Must have sensitivity to cultural differences, HIV/AIDS, minority health, sexual practices, and work with LGBTQ+ communities.
• Community-based case management experience desired.
• Proficiency in Microsoft Office and computer applications required.
• Reliable transportation, driver's license and auto insurance proof required.
• Knowledge and adherence to social work standards and ethics.

Responsibilities

• Provide high quality case management to clients and families by linking to medical services, insurance, Ryan White benefits, and housing resources.
• Conduct psychosocial assessments at intake and periodically update; complete required documentation.
• Develop, monitor and adjust individual case plans addressing services within Equitas Health and external providers.
• Identify and access financial assistance programs and assist with paperwork to maintain eligibility.
• Represent Equitas Health in the community and collaborate with other providers to support client needs.
