Noble Community Clinics is looking for a detail-oriented and patient-focusedHealth Information Management (HIM) Specialist to join our team. In this role, you will help ensure patient health information is accurate, complete, secure, and available to authorized individuals across our clinics.
The HIM Specialist plays an important role in protecting patient privacy while providing timely and accurate release of information services. This position works with electronic health records, medical records, providers, staff, patients, and external organizations to ensure health information is properly managed and maintained in accordance with HIPAA, state and federal regulations, and Noble policies.

Health Information Management & Medical Records
Coordinate, manage, and maintain patient medical records across Noble Community Clinics.
Ensure patient records are complete, accurate, and maintained according to organizational and regulatory standards.
Support electronic health record (EHR) workflows, records management, confidentiality, security, and health information exchange.
Monitor reports and records management activities and assist with HIM-related projects and initiatives.
Release of Information & Compliance
Process requests for patient records and release of information (ROI) accurately and within required timeframes.
Verify that requests and authorizations are valid and comply with HIPAA, state and federal regulations, and Noble policies.
Provide authorized medical information for insurance companies, audits, legal requests, and other approved purposes.
Protect patient privacy and ensure protected health information is only released to authorized individuals.
Coordinate legal records requests, including subpoenas, attorney requests, court orders, law enforcement requests, licensing boards, and regulatory agencies.
Coordinate the timely completion of death certificates.
Electronic Health Records & Document Management
Scan and accurately index paper documentation into the electronic health record.
Integrate electronic information from multiple sources into the patient record.
Process faxed, mailed, and electronic medical records requests.
Maintain accurate documentation in accordance with organizational and regulatory requirements.
Auditing & Data Integrity
Perform medical record audits as assigned.
Assist with data validation and record accuracy initiatives.
Monitor reports to identify incomplete records or compliance concerns.
Support quality improvement efforts related to health information management.
Identify and report discrepancies, trends, or concerns related to patient records and documentation.
Respond to telephone calls and inquiries related to medical records and health information.
Provide professional and responsive customer service to patients, providers, staff, and external organizations.
Develop effective working relationships with providers, leadership, and staff across the organization.
Collaborate with internal departments to ensure medical records and health information are properly maintained.
Required:
High school diploma or equivalent.
At least1 year of experience in Health Information Management (HIM) or medical records in a healthcare setting.
Strong attention to detail and ability to maintain confidentiality.
Ability to manage sensitive and protected health information appropriately.
Excellent organizational, communication, and customer service skills.
Ability to work independently while collaborating effectively with a variety of departments and team members.
CPR certification within six months of hire.
Preferred:
Associate degree or certificate in Health Information Management, Health Information Technology, or related coursework.
RHIA or RHIT certification.
Experience working withOCHIN Epic.
Knowledge or experience working in aFederally Qualified Health Center (FQHC).
Experience with electronic health records and medical record systems.
