Qualifications

Responsibilities

• Assemble and organize patient records in the correct order and sequence.
• Analyze records to identify missing documentation and flag incomplete entries.
• Prepare patient charts by gathering and verifying documentation from clinical encounters.
• Notify providers of incomplete or missing dictation to ensure timely completion.
• Monitor documentation completion to maintain compliance with record standards.
• File, scan, and index health records into the EMR system per policies.
• Respond to internal and external health record requests in compliance with HIPAA and policies.
