Cook Children’s Health Care System is seeking a HIM Forms Governance Coordinator within the HIM-Quality department. This role helps manage enterprise forms governance, reviews, maintenance, and lifecycle management of clinical and administrative forms to promote standardization and regulatory compliance across the health system.

Under HIM leadership, you will collaborate with the HIM Project Coordinator and stakeholders to maintain the centralized forms repository, support governance workflows,

• 2 years of experience in HIM, healthcare operations, document management, or administrative coordination required.
• 4 years of experience in HIM, healthcare operations, document management, or administrative coordination required may be substituted for the required education.

Summary: The HIM Forms Governance Coordinator is a key member of the Health Information Management team, responsible for supporting the organization's enterprise forms governance and management program. This position coordinates the development, review, maintenance, and lifecycle management of clinical and administrative forms to promote standardization, regulatory compliance, and effective forms management across the health system. Working under the direction of HIM leadership and in collaboration with the HIM Project Coordinator and organizational stakeholders, the coordinator plays a critical role in maintaining the centralized forms repository, supporting governance workflows, and ensuring forms remain current, accurate, appropriately approved, and available to support operational and patient care needs.
